    Customer Responsibilities


     

    Cord Blood & Cord Tissue Banking

    • Complete Required Documents
      Electronic documents were emailed to you from your Family Clinical Consultant via DocuSign. The forms are required to complete the banking process.
       
    • Collection Kit 
      Bring your ViaCord Collection Kit to the hospital on the day of your baby's birth and give it to your medical team to perform the cord blood and cord tissue collection. Call ViaCord  866-668-4895 when the collection is complete to arrange for pick up. 
       


    Genomic Testing
    Testing will not begin until the following customer responsibilities have been completed. As a reminder, ViaCord works with Genome Medical to provide expert guidance throughout the entire testing process – from start to finish.     • Review and Sign your ViaCord Service Agreement
      The electronic form was emailed to you from your Family Clinical Consultant via DocuSign. The electronic Form must be completed before we can process your order.  
       
    • Activate your Genome Medical account
      Look for an email from Genome Medical with the subject line: ViaCord Customers: Schedule your required pre-test consultation with Genome Medical. If you cannot locate your email, please call Genome Medical at 877-688-0992.
       
    • Schedule Your Required Pre-Test Consultation
      A Genome Medical professional will discuss your testing journey and any questions. Scheduling is easy! Watch this short video to see how 
       
    • Saliva Sample (only for tests requiring saliva samples) 
      If your DNA test requires an at home saliva sample, your collection kit will be shipped to the address on your order. You'll receive kit shipped notification email with tracking information.  Easily collect and return your sample using the simple instructions and prepaid shipping envelopes in the kit.


    Cancellation Fee
    A $150 non-refundable fee will be charged to your credit card if ViaCord’s Cord Blood Banking service is discontinued prior to delivery and an additional $150 cancellation fee will be charged for genomic testing if cancelled after the processing of documents by Genome Medical. 

    ViaCord Responsibilities

    • Cord Blood Collection Kit Shipment 
      Collection kits are shipped to arrive by week 27 in the pregnancy. If you are due less than 13 weeks away, the kit will arrive shortly.
       
    • Saliva Sample Kit (only for tests requiring saliva samples)
      Your DNA saliva collection kit will be shipped to the mailing address on your order. You'll receive an e-mail notification with tracking information upon shipment. Easily collect and return your sample using the simple instructions and prepaid shipping envelopes in the kit.
       
    • Stem Cell Collection Results
      ViaCord will send your newborn's cord blood collection results report approximately 6-8 weeks after the stem cells have been processed and stored at ViaCord's Lab. 
       
    • Genomic Test Results 
      Genome Medical will send your test results in about 8 weeks. They'll review them with you and discuss any questions or next steps

    Payment: What to expect

    The credit card on your account will be charged automatically at the time of your delivery and upon successful processing and storage of your newborn’s stem cells. One charge will appear on your credit card statement. The charge represents your cost for all of your ordered products. If you have opted for one of our flexible monthly payment plans, your payment will be deducted each month on the date of your child’s delivery. Please note your cord blood annual storage fee will be billed automatically to the credit card listed on the 10th of the month your child was born. A statement will be sent to your home address.
